Nurse Practitioner

We are seeking a highly skilled and compassionate Registered Nurse to join our team in the Children's Continuing Care service. As a key member of our community nursing team, you will be responsible for delivering high-quality care to children and young people with complex health needs in their own homes and community settings.
As a Registered Nurse in this role, you will work closely with families, carers, and other healthcare professionals to develop and implement individualized care plans that meet the unique needs of each child. You will also be responsible for assessing and monitoring patient conditions, arranging diagnostic tests, and recognizing and responding to emergency situations.

Key Responsibilities- Deliver high-quality care to children and young people with complex health needs in their own homes and community settings.
- Develop and implement individualized care plans that meet the unique needs of each child.
- Assess and monitor patient conditions, arrange diagnostic tests, and recognize and respond to emergency situations.
- Work closely with families, carers, and other healthcare professionals to provide comprehensive care.
- Participate in ongoing training and development opportunities to enhance your skills and expertise.
- Registered Children's Nurse qualification.
- Minimum of 6 months paid experience in a Registered Nurse Children's Continuing Care (Community) environment within the last 2 years.
- Valid UK driving license and access to a car.
- Ability to work flexible hours, including weekends and Bank Holidays.
