Bridge Street Surgery is seeking an experienced practice nurse to join our team on a part-time basis. The successful candidate will be responsible for managing chronic diseases, including diabetes and respiratory conditions.
Main Responsibilities- Manage chronic diseases, including diagnosis, monitoring, and development of individual management plans.
- Experience in diabetes and respiratory disease management.
- Check compliance with and adherence to appropriate treatments using a holistic patient-centred approach.
- Identify the impact of treatment and implement or recommend changes as appropriate.
- Assess routine wounds, including trauma.
- Undertake patient health checks.
- Recognize issues and gather sufficient information to refer patients to relevant services.
- Undertake monitoring tasks, such as smears, and provide advice as appropriate.
- Administer travel clinic injections and provide guidance in accordance with guidelines.
- Immunizations for adults and children.
- Assist the general practitioner in undertaking simple procedures.
We are looking for a candidate with the following qualifications, registrations, skills, and experience:
- A relevant nursing degree.
- Current NMC registration with an active PIN.
- Fundamentals of Practice Nursing course (General Practice Nurse Foundation Programme) or equivalent experience or qualifications.
- Competence to undertake diabetic clinics and reviews, holding relevant certificates.
- Experience in supporting patients with diabetes.
- Experience of other chronic diseases, such as asthma and COPD, is desirable.
- Knowledge and experience of supporting long-term conditions.
- Insight and understanding of current issues in nursing and the wider NHS.
