About the Role: We are seeking a dedicated and proactive Social Care Support Officer to join the Intermediate Care & Allocations Team within Adult Social Care at Lancashire County Council.
The Intermediate Care Allocation Team comprises a group of social care professionals who collaborate with hospitals and community services. Our mission is to facilitate safe and efficient hospital discharges while conducting comprehensive social care assessments. We strive to prevent unnecessary hospital admissions by promptly evaluating the care needs of our service users. As a Social Care Support Officer, you will engage with various services to ensure optimal outcomes for individuals both in hospital and at home, assessing the needs of service users and their caregivers.
