Main Duties of the JobWe are seeking to appoint experienced mental health professionals to work in our community teams, providing assessment, treatment, and care coordination for older adults with complex mental health needs.
About the RoleAs a Community Practitioner, you will work as part of a multi-disciplinary team to provide high-quality care and support to older adults in their own homes or in community settings.
Key Responsibilities- Undertake comprehensive assessments of older adults with complex mental health needs
- Develop and implement evidence-based treatment plans to support recovery and well-being
- Act as care coordinator for complex cases, working with other professionals to ensure seamless care
- Supervise and support junior staff, including students and newly qualified professionals
- Participate in multi-disciplinary team meetings and contribute to the development of care pathways and policies
We are looking for experienced mental health professionals who possess the following skills and qualifications:
- Professional qualification and current registration as a mental health nurse, AHP, social worker, or clinical psychologist
- Mentor training and experience of supervising junior staff
- Post-registration training in clinical/psychological interventions and skills relevant to the post
- Proven track record of post-registration clinical experience in mental health
- Training in CPA, Safeguarding, MAPPA, and Child Protection
- Up-to-date knowledge and experience of relevant legislation
