About the Role
We are seeking a skilled and compassionate Registered Nurse to join our team as a Community Care Specialist. As a key member of our community care team, you will be responsible for assessing and reviewing individual care packages, providing practical training to care workers, and clinical supervisions and instructions using your clinical expertise and guidance.
Key Responsibilities
- Assess and review individual care packages to ensure high-quality care is delivered to adults, children, and young people with varying conditions.
- Provide practical training to care workers and clinical supervisions and instructions using your clinical expertise and guidance.
- Work with our healthcare workers at the point of care, which may include home, school, community, and day centers.
- Interact with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments.
- Participate in our paid phone support On Call Rota on a rotational basis.
Requirements
- Registered Nurse with experience in ICU/HDU or complex care.
- Self-motivated, enthusiastic, caring, empathetic, and compassionate individual.

- A competitive salary up to £43,940.
- A driving license is required.
- A Monday to Friday, 9am to 5pm schedule with no weekend or bank holiday working.
