Job Summary:

We are seeking a highly skilled Senior Clinical Practitioner to join our Frailty Team at Maple View Medical Practice. The successful candidate will be responsible for participating in clinical assessments of patients with complex healthcare needs, developing and evaluating care management plans, and contributing to multidisciplinary team discussions for effective care.

Key Responsibilities:

  • Conduct comprehensive clinical assessments of patients with complex healthcare needs.
  • Develop and implement individualized care management plans to meet the unique needs of patients.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
  • Prescribe medications and review their effectiveness in conjunction with patients' healthcare providers.
  • Maintain accurate and up-to-date patient records, ensuring compliance with regulatory requirements.
