We are seeking a highly skilled Clinical Pharmacist to join our team at Alliance for Better Care CIC. As a Clinical Pharmacist, you will play a vital role in improving patient outcomes and quality of care in our general practice settings.
Key Responsibilities- Provide expert clinical leadership on medicines optimisation and quality improvement initiatives
- Develop and implement strategies to improve prescribing practices and reduce medication errors
- Work closely with general practice teams to ensure seamless integration of pharmacy services
- Conduct clinical medication reviews and provide recommendations for senior pharmacists, nurses, and GPs
- Provide patient-facing clinics and telephone support for patients with medication-related queries and concerns
- Act as a source of medicines information for practice teams and patients
- Reconcile medicines following discharge from hospitals, intermediate care, and care homes
- Set up and manage systems to ensure continuity of medicines supply to high-risk patient groups
- Produce and implement a practice repeat prescribing policy in line with recommendations from the Medicines Management Team
- Manage the repeat prescribing reauthorisation process and review patient requests for repeat prescriptions
- Support frail patients with their medication, including medication review and patient identification
- Undertake clinical audits of prescribing and provide feedback to practice teams
- Work with the Medicines Management Team to support the implementation of local and national guidelines
- Liaise with hospital colleagues to ensure seamless transfer of care
- Provide education and training to primary healthcare teams on therapeutics and medicines optimisation
- Ensure practice compliance with CQC standards related to medicines
- Support public health campaigns and provide specialist knowledge on public health programmes
- Minimum of 2 years' experience post-qualification as a pharmacist
- Therapeutic and clinical knowledge and understanding of evidence-based medicine and healthcare
- Good knowledge of service developments in medicines, prescribing, NHS policy, and other relevant policy directives
- Excellent understanding and implementation of legal, ethical, professional, and organisational policies/procedures and codes of conduct
- Good understanding of patient-centred care and its impact on care quality and outcomes
- Good understanding of patients' roles in decisions about prescribed medicines and supporting adherence as per NICE guidance
- Experience of multi-disciplinary team working and committees/working groups
- Drug information experience and experience in writing complex and high-quality reports
- Experience in audit, research, and analysis of highly complex information related to improving clinical quality
- Experience in supervising staff
- Experience of independent patient consultation, review, and recommendations/prescribing
