We are seeking a dedicated and compassionate individual to join our Pan London Rough Sleeping Team as an Assessment and Reconnection Worker. As a key member of our team, you will play a vital role in supporting individuals at risk of rough sleeping and making a positive impact on their lives.

Key Responsibilities
  • Provide personalized case management services to clients, developing tailored action plans to address their unique needs.
  • Build and maintain effective relationships with clients, fostering trust and supporting them throughout their recovery journey.
  • Stay up-to-date with relevant legislation, including welfare benefits and housing laws, to ensure informed support.
  • Work collaboratively with colleagues to ensure seamless service delivery and effective communication.
  • Participate in a weekly rota, including early, mid, and late shifts, with occasional evening and weekend work.
