Job Summary

We are seeking a skilled Social Worker to join our Children with Disabilities Team. As a key member of our team, you will work closely with children and young people with complex needs, providing support and guidance to help them achieve their full potential.

Key Responsibilities
  • Assess the needs of children and young people with disabilities, developing personalized care plans to promote their independence and well-being.
  • Coordinate access to services and resources that support the needs of children and young people with disabilities.
  • Work collaboratively with other professionals, including healthcare providers and education specialists, to ensure a comprehensive approach to supporting children and young people with disabilities.
Requirements

To be successful in this role, you will need:

  • A degree in Social Work or a related field.
  • Experience working with children and young people with disabilities.
  • Strong assessment and planning skills, with the ability to develop and implement effective care plans.
  • Excellent communication and interpersonal skills, with the ability to work collaboratively with a range of professionals and stakeholders.
