Position Overview
An exceptional opportunity has emerged for a seasoned Nurse Practitioner or Paramedic to assume a pivotal role within a dynamic and challenging community-based Intermediate Care Team. This team has been dedicated to providing healthcare in the comfort of patients' homes for an extensive period. Our commitment lies in delivering high-quality care, minimizing unnecessary hospital admissions, and facilitating swift discharges to promote independent living through collaboration with various community services and social care providers.
Our Intermediate Care Team is at the forefront of clinical innovation, consistently developing new patient pathways in partnership with other organizations and utilizing advanced remote monitoring technologies to manage a Virtual Ward. The team's emphasis on preventing admissions and optimizing patient flow has been crucial in allowing patients to remain in their homes whenever feasible. Recently, we have intensified our focus on enhancing our rapid response capabilities and establishing pathways with emergency services.
We are in search of a proactive, adaptable, and experienced Nurse Practitioner or Paramedic with a broad range of expertise in health conditions, including acute and chronic care for the elderly. The ideal candidate will contribute to the ongoing development of our community-based team.
Key Responsibilities
The successful candidate will demonstrate confidence in providing effective clinical leadership to the team and thrive under pressure. They must possess advanced clinical assessment skills, be proficient in prescribing, and exhibit strong problem-solving abilities.
The role requires leading and coordinating compassionate, high-quality, and evidence-based care by supporting the Clinical Lead and collaborating with Allied Health Professional Leads, other Intermediate Care Team members, and system partners to ensure the safe and effective delivery of services during operational hours, which may include out-of-hours care.
Exceptional communication skills are essential for fostering collaboration within the multidisciplinary team, including the therapy services within the Intermediate Care Team. The candidate will provide strong clinical leadership that encourages autonomy and professional development among the workforce.

Qualifications and Experience
Essential
- NMC Adult Nursing Qualification or HCPC Paramedic Qualification
- Non-medical prescribing or in progress
- At least 2 years of clinical experience at Band 7 or equivalent
- Leadership/management qualification
- Advanced Practitioner qualification or similar
Desirable
- Experience in project management
- Ability to administer IV antibiotics and train others
- Experience in clinical rapid response, triage, or assessment in community settings
