Job Summary

We are seeking a highly skilled Paediatric Diabetes Specialist Nurse to join our esteemed team at Calderdale and Huddersfield NHS Foundation Trust. This is an exciting opportunity to work with a multidisciplinary team that has a proven track record of delivering exceptional care to children and young people with type 1 diabetes.

Main Responsibilities
  • Provide evidence-based specialist care to children, young people, and their families with Type 1 and Type 2 diabetes in Calderdale and Huddersfield.
  • Manage a caseload of patients and make clinical decisions on a daily basis, involving altering insulin doses, offering telephone support, and supporting school staff and other professionals.
  • Attend multidisciplinary clinics on a weekly basis and participate in regional Nursing Network meetings.
  • Support children that require safeguarding intervention and provide continued support and information to children on pump therapy.
  • Provide structured education to families and relevant professionals involved in the care of children and young people.
  • Compile audit data and contribute to the improvement of diabetes care services.

Requirements
  • Current NMC registration and professional or clinical qualification relevant to the speciality.
  • Experience of caring for children and young people with diabetes and evidence of continual professional development.
  • Safeguarding Children Level 3 training and knowledge of Diabetes NICE guidance and diabetes best practice tariff.
  • Excellent communication skills and ability to work autonomously and as part of a team.
