About the Role

We are seeking a skilled and compassionate Care Practitioner to join our team at Woodlands Neurological Rehabilitation Centre in York. As a key member of our multidisciplinary team, you will play a vital role in ensuring the safe and effective delivery of care to our patients.

Key Responsibilities
  • Ensure the safe running of the shift, working in partnership with the nurse in charge
  • Engage with care planning, managing the administration, storage and ordering of medications
  • Support the multidisciplinary team with patient personalized timetabling
  • Ensure the environment is well maintained and participate in auditing and implementation of change
What You'll Be Working

Our standard working hours are 42 hours per week, Monday to Friday. However, we are flexible and willing to discuss alternative arrangements to meet the needs of the service.

What You'll Be Doing
  • Deliver high-quality care and rehabilitation services that promote independence, dignity, privacy and safety
  • Work in support of qualified staff and allied healthcare professionals in the assessment, planning, implementing and evaluation of care
  • Lead the rehabilitation team in ensuring holistic care is delivered and standards of care are enhanced
  • Communicate effectively with the team, other healthcare professionals and patients and their families
  • Ensure high standards are maintained in a safe environment that promotes equality and sensitivity for all individuals
Requirements
  • Educated to NVQ 3/4 or equivalent, with additional training to diploma level
  • Experience of the specialty area is desirable, but not essential
  • Grade C or above in GCSE Maths and English is desirable
