As a Health Visitor with Aneurin Bevan University Health Board, you will play a vital role in promoting the health and wellbeing of families within our defined population. Your primary objective will be to increase family resilience, improve population health outcomes, and reduce health inequalities while safeguarding the wellbeing of children.
Main Responsibilities- Manage a caseload within a defined population, utilizing the All Wales Health Visiting Acuity tool to ensure safe staffing levels and support colleagues during fluctuations in workforce and activity.
- Provide a Health Visiting service for a defined caseload and offer cover for other health visitors during periods of leave.
- Develop and maintain health profiles of an identified population to identify health needs and influence service plans.
- Contribute to local partnership plans and promote new opportunities for integrated working.
- Share responsibility for chairing Health Visiting Forums/team meetings and contribute to the development of local policies and guidelines.
This is an exciting opportunity for an experienced and enthusiastic Health Visitor to join our dynamic multiagency team. As a Health Visitor with Aneurin Bevan University Health Board, you will have the opportunity to work within a very established team, providing the Healthy Child Wales Programme to all families within the area.
The ability to speak Welsh is desirable for this post, and Welsh and/or English speakers are equally welcome to apply.
