Position Overview

The successful candidate will oversee the effective operation of both physician-led and nursing-led outpatient clinics, ensuring the delivery of expert nursing care to patients undergoing day case treatments. Responsibilities include evaluating patient care requirements and formulating, executing, and assessing care plans independently.

Key Responsibilities

  • Provide safe, proficient, and empathetic care to renal patients during outpatient consultations and day-case treatments.
  • Administer haemodialysis for patients with advanced kidney disease.
  • Function autonomously as a proactive and adaptable team member.
  • Act in a leadership capacity during the absence of senior personnel to maintain patient care continuity in the Renal Outpatient department.
  • Facilitate and assist in various medical and nursing-led clinics.
  • Conduct comprehensive patient assessments and coordinate referrals to multidisciplinary or community teams as necessary.
  • Delegate tasks appropriately and serve as a professional role model within the nursing team.

Job Duties

Plan and deliver care and assessments for renal patients utilizing proficient skills in areas such as venepuncture, cannulation, and intravenous drug administration. Review patient care needs within the department while supporting Healthcare Assistants in clinic operations. Accurately interpret test results to inform appropriate actions and referrals to the multidisciplinary team.

Engage actively in the development, implementation, and evaluation of care programs with a flexible approach, involving all members of the multidisciplinary healthcare team to promote patient-centered care.

Provide follow-up care via telephone for patients and caregivers, keeping them informed and supported while at home. Offer emotional and psychological support to patients and families navigating diagnoses and significant decisions regarding renal treatment and dialysis, referring to other professionals and agencies as necessary. Facilitate patient and family understanding of renal failure and its treatment, ensuring access to additional psycho-social support as required.

Take the nursing lead in caring for specific patient groups, such as those awaiting kidney transplants, managing their test results and follow-up care through phone consultations and nurse-led pre and post-procedure care for renal outpatients undergoing day procedures and intravenous infusions, monitoring vital signs and escalating concerns to the medical team as indicated.

Actively participate as a collaborative team member, contributing to the efficient operation of patient transfers between departments in accordance with Trust guidelines and internal policies, ensuring safe management of day case conversions to inpatient care.

  • Proficient in phlebotomy and venepuncture techniques
  • Proficient in cannulation techniques
  • Proficient in intravenous therapy

  • Knowledge of General Nephrology
  • Experience working effectively within a multidisciplinary team

Desirable

  • Experience as a Senior Nurse in a renal setting
  • Skills in haemodialysis and/or peritoneal dialysis
  • Experience in caring for patients on the transplant waiting list and providing post-transplant follow-up care
  • Understanding of renal anaemia treatment
  • Familiarity with the Renal CV5 database
  • Knowledge of non-transplant immunosuppression treatments
