Position Details:

Location: Various locations across the community service.

As a key member of our multidisciplinary team, you will be responsible for delivering exceptional, patient-centered care, ensuring the highest standards of practice within our division.

Candidate Profile:

The ideal candidate will possess extensive experience in mental health services, demonstrating strong teamwork abilities and effective communication skills to foster relationships across clinical teams. You should be resilient, capable of thriving under pressure, and possess excellent problem-solving skills. A valid NMC registration is essential.

Key Responsibilities:

  • Strategically plan and allocate staff based on competencies and patient needs.
  • Develop, implement, and evaluate outcome-focused care plans that reflect individual preferences.
  • Maintain effective communication with all members of the multidisciplinary team, managing care environments positively.
  • Build and sustain trusting relationships with patients, families, and caregivers, addressing their needs and concerns.
  • Ensure accurate and up-to-date patient records, documenting risks and interventions.
  • Utilize reflection and supervision to enhance learning and improve patient outcomes.
  • Provide and receive clinical supervision as part of professional development.
  • Contribute to the division's delivery of specific outcome measures.
  • Conduct necessary audits in alignment with Clinical Governance strategies.

Physical Requirements:

Candidates must meet physical capability standards, including the ability to sit, stand, and kneel for extended periods, as well as transition smoothly between these positions.
