About the Role

We are seeking a highly skilled and compassionate nurse to join our Virtually Assessed Pathway (VAP) team at Velindre Cancer Centre. As a VAP Clinic Nurse, you will play a vital role in assessing patients who are due to receive chemotherapy or Systemic Anti-Cancer Therapy (SACT) in the next 2-3 days.

Key Responsibilities
  • Work collaboratively with patients and their carers to assess their needs and develop personalized care plans.
  • Liaise with the multi-disciplinary team to ensure seamless care coordination.
  • Act as the named nurse/keyworker for designated individuals, providing ongoing support and guidance.
  • Ensure patients receive care according to plan, delegating tasks to other team members as necessary.
