The Mental Health Practitioner is responsible for evaluating the appropriateness of new referrals, ensuring compliance with departmental referral guidelines.
Key Responsibilities- Direct unsuitable clients to the appropriate service or back to the referral source as needed, or escalate treatment to a higher intensity therapy.
- Develop personalized treatment plans that cater to the unique needs of each individual.
- Maintain an agreed-upon activity contract that specifies the number of client interactions and clinical sessions conducted weekly, aimed at reducing waiting times and ensuring accessible treatment delivery.
- Assess and manage risks associated with patients who may pose a threat to themselves or others, in accordance with established Service and Trust protocols.
- Fulfill all data collection requirements within the service.
- Incorporate work and employment-related issues into the overall therapeutic process.
- Uphold client confidentiality at all times, adhering to Service and Trust policies.
- Stay informed about advancements in the treatment of common mental health issues.
- Participate in regular Case Supervision and Reflective Practice Supervision as agreed with the Locality Manager and Clinical Lead.
- Contribute to the enhancement of best practices within the service.
