Role Overview

The Mental Health Practitioner is responsible for evaluating the appropriateness of new referrals, ensuring compliance with departmental referral guidelines.

Key Responsibilities
  1. Direct unsuitable clients to the appropriate service or back to the referral source as needed, or escalate treatment to a higher intensity therapy.
  2. Develop personalized treatment plans that cater to the unique needs of each individual.
  3. Maintain an agreed-upon activity contract that specifies the number of client interactions and clinical sessions conducted weekly, aimed at reducing waiting times and ensuring accessible treatment delivery.
  4. Assess and manage risks associated with patients who may pose a threat to themselves or others, in accordance with established Service and Trust protocols.
  5. Fulfill all data collection requirements within the service.
  6. Incorporate work and employment-related issues into the overall therapeutic process.
  7. Uphold client confidentiality at all times, adhering to Service and Trust policies.
  8. Stay informed about advancements in the treatment of common mental health issues.
  9. Participate in regular Case Supervision and Reflective Practice Supervision as agreed with the Locality Manager and Clinical Lead.
  10. Contribute to the enhancement of best practices within the service.
