- Delivering professional nursing advice on Continuing Care and Personal Health Budgets to various healthcare organizations.
- Collaborating with the continuing care team to ensure timely and comprehensive reviews are conducted, adhering to national frameworks and timelines.
- Facilitating effective communication and coordination between multidisciplinary teams involved in patient care.
- Providing expert knowledge and guidance on Continuing Care and PHB policies and procedures.
The ideal candidate will possess a strong clinical background, coupled with a comprehensive understanding of Continuing Care and PHB principles. Excellent communication, interpersonal, and organizational skills are essential for success in this role.
