You will play a key role in supporting adults referred by GPs with a wide range of mental health needs, providing timely assessments, interventions, and care coordination within the community. Purpose of the Role The postholder will provide specialist mental health nursing within the Primary Care Liaison Service. You will support adults referred directly by GPs, offering rapid assessment, clinical triage, brief interventions, and risk management. The role aims to ensure timely, effective support, preventing unnecessary escalation to secondary mental health services. What You Will Do Carry out rapid, comprehensive mental health assessments for adults referred through primary care. Screen for severity, complexity, safeguarding issues, risk factors, and appropriateness for PCLS intervention. Provide clear clinical recommendations, including brief interventions, signposting, or escalation where necessary. Deliver evidence-based interventions such as solution-focused approaches, brief CBT-informed strategies, motivational interviewing, and emotion regulation work. Support individuals experiencing anxiety, depression, trauma, personality-related difficulties, and mild psychotic symptoms. Hold a small caseload, developing, reviewing, and updating personalised care and safety plans. Complete thorough risk assessments, including risks related to self-harm, suicide, domestic abuse, vulnerability, or psychotic experiences, and create risk management plans accordingly. Work collaboratively with GPs and primary care networks, offering mental health guidance and participating in joint reviews and meetings. Engage closely with psychologists, psychiatrists, support workers, recovery teams, and wider MDT colleagues. Maintain accurate, detailed clinical records and contribute to audits and quality improvement initiatives. Uphold professional standards, participate in clinical supervision and CPD, and promote trauma-informed, compassionate care.
