**Job Title: Social Prescriber**

**Hours: 37.5 hours weekly (Monday to Friday)**

**Salary: £22741.00 per annum**

**Accountable to: Social Prescribing Project Managers**

**Responsible to Social Prescribing Services Manager**

**Base: Age UK North Yorkshire Coast and Moors and Advocacy Alliance Offices, community and GP Surgeries covering Central Scarborough and Eastfield / Scarborough, Filey and surrounding areas**

Social Prescribing is a non-clinical, holistic intervention offering individuals the opportunity to focus on what matters to them. As a Social prescriber covering central Scarborough or Scarborough and Filey areas, you will be part of a dynamic, creative and professional team. You will enhance individual’s wellbeing through Social Prescribing, working co-productively to produce meaningful solutions for those who have been referred into the service.

**Purpose of the role**

Social prescribing empowers people to take control of their health and wellbeing through referral to non-clinical workers, who give people time to focus on ‘what matters to me’ and take a wider and holistic view to people’s health and wellbeing.

**Social prescriber**
- Helps people to identify issues that affect their health & wellbeing, and empower the individual to develop a simple personalised plan
- Support people by connecting them to non-medical, community-based activities, groups and services that meet their practical, social and emotional needs, including specialist advice services and arts and culture, physical activity, and nature and green based activities

**Salary**: £22,741.00 per year
