We currently have an exciting opportunity to join the Discharge Team on a permanent basis. Working as a Discharge Co-ordinator you will as part of a wider team manage the daily process of pro-active Discharge Planning within Forth Valley Royal Hospital to support the safe and timely transfer / discharge of patients from the Acute Hospital.

All applicants should possess the ability to prioritise fluctuating workloads, remain focused, work across professional boundaries and be flexible in an extremely busy environment. You must be able to demonstrate good organisational skills, excellent communication skills, problem solving ability, a commitment to continuing professional development and supporting education of ward teams and students.

This post is open to 1st Level Registered Nurses and AHP registered with HCPC

The service covers 7 days a week 7am-6pm, so weekend working on a rota basis is part of the role.
