**Job Description - Mental Health Care Coordinator - **_**Interview date: 19th September**_

**An exciting opportunity has arisen for an enthusiastic and self-motivated Mental Health Care Coordinator to join our team at Bramhall and Cheadle Hulme PCN (BaCH).**

BaCH PCN comprises 5 GP Practices and covers a population of circa 58,000 patients. You will join a growing team of Care Coordinators who cover specialist areas, such as Cancer, Frailty, Learning Disability, Mental Health and Safeguarding.

You will have your own caseload of identified patients and be required to ensure that their changing needs are addressed by considering local priorities, health inequalities and/or population health management risk.

This role will specifically work with patients with severe mental illness and will entail building positive relationships with this cohort of patients, completing their annual health checks in practices and with home visits where required and offering specific ongoing support where needed.

This is fixed term full time position to cover maternity leave from October 2023 to April 2024 (and could be extended further).

We are looking for someone who has previous experience of working within the field of mental healthcare and who has administration and organisation skills. You will need to be a car driver with access to a suitable vehicle for work purposes.

- Willingness to undertake venepuncture and basic observations training
- Understanding of equal opportunity and diversity issues
- Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own home.

- Awareness and knowledge of vaccine and immunisation programmes
- Understanding of role of patient advocate.

**Qualifications**

Essential
- English and Maths to GCSE Grade C or above or equivalent.

Desirable
- NVQ level 3 or working towards
