Harrow Community Services is seeking to appoint a professional, competent, enthusiastic, and experienced Physiotherapist/Occupational Therapist to join us to build on existing models of care and introduce integrated models of care in the Borough.

The transformation plan over the next 12 months includes further integration across primary, acute care, social care, voluntary sector and patient groups, with a greater emphasis on population health through collaborative working with neighbourhood teams

We are therefore looking to appoint a leader and is able to manage a large team of Therapists, to deliver good quality care and one who reflects the core values of the Trust

The post is for a motivated and committed Clinical Lead to join our Integrated Care Unit in the Borough of Harrow.
- The post holder is responsible for the operational and clinical management of the patient care services within the clinical specialty, including accountability for the patients' experience of care and the clinical environment.
- The post holder will assess and treat own caseload of patients, take an active role in the development of the Therapy service and assist others in prioritising managing complicated and challenging conditions. They will arrange formal and informal meetings, training sessions and appraisals to achieve this.
- To provide strong clinical and operational leadership to staff and patient care in accordance with Harrow ICT model
- The focus of which is the continuing development of a seamless integrated service between the acute trusts, community services, PCNs, care homes, social services and voluntary sector.
