The Value Based Health Care and Pathways team within Betsi Cadwaladr University Health Board is recruiting from local practising GP’s to meet the demand of its exciting and growing pathway portfolio.
Working across primary, community and secondary care, the post holder will provide clinical leadership, advice and guidance to progress the implementation of the on-line platform as part of an All Wales National Project.
HealthPathways is a localised online platform used by Primary Care teams to inform and join up service provision. There is particular emphasis on the Quadruple Aims of improving population health and wellbeing, improving quality and accessibility, improving patient experience, ensuring the development of a sustainable workforce and reducing harm, waste and variation, all of which add real value to the population of North Wales.

Working as part of the Care Pathways Team the post holder will work closely with the Transformation and Improvements Department and key stakeholders to successfully lead, develop and implement a number of pathway redesign projects across the health board.
The post holder is central to the delivery of pathway redesign; they will act as an educator, advisor and facilitator. The role is to foster close working relationship between the primary, community and secondary care teams, which requires a clinical leadership model of service change to effectively influence and engage diverse groups of stakeholders from within the Health Board and the local community in challenging change programmes.
