The post holder will work as an integral part of the Children’s Community Learning Disabilities Team. Participating in the effective delivery of individualised assessments, planning, implementation and evaluation of children’s care. Delivering therapeutic intervention to pre-school children who have learning disabilities, physical disabilities and complex healthcare needs. Leading and supporting a small team of Clinical support Workers. Assisting the Registered Nurse and the wider multi-agency team in organising structured activities to meet each child’s individual needs, in both the assessment and intervention nursery.

To be a Keyworker for an identified group of children.

Preparation and clearing away of the nursery.

Participation in the developmental activities for the children.

To assist nursing and therapy staff in co-ordinating and implementing activities.

To advise and assist in the recommendation and preparation of appropriate play activities for the children attending Nursery.

To ensure the planning, implementation and evaluation of individual activity plans with Keyworkers.

To assist the Registered Nurse in planning medical care plans.

To ensure completions of the initial assessments for all children and support with report writing.

To attend to the personal care needs of the children as required (this may require frequent moving and handling).

To assist the Registered Nurse with admissions, discharges and session planning.

To ensure accurate record keeping in line with Team and Trust requirements and assist with documentation audits.
