Duties, Responsibilities and Key Results Provides care for patients with palliative care needs in accordance with the patients care plan. Communicates with patients carers and health professionals in a professional and appropriate manner; whilst being aware of when the matter is to be forwarded to a trained member of staff. Recognises changes in symptoms and conditions and notifies other health care professionals as necessary. Attends to the basic hygiene, toiletry and feeding needs of the patient, whilst reporting any concerns or conditional changes to a trained member of staff.

Provides support to patients carers or family members. Cares for the body after death and supports family members / carers after death. Updates patients records, and hands over given care appropriately. Is competent in recording the patients vital signs, including blood glucose monitoring (as per training) and reports adverse results to the trained member of staff.

Is able to use all relevant equipment competently including manual handling and pressure relieving aids etc. Practices in accordance with St Davids Hospices health and safety policies and adheres to fire regulations; and maintains own personal safety accordingly. Attends annual mandatory training. Maintains own conduct in accordance with St Davids Hospices Employee Handbook and Hospice policies.

Participates in own personal development initiatives through training and education. Participates in an annual individual performance review. Is I.T. literate, or working towards sufficient literacy for their working needs.

In time, training for additional skills may be offered e.g. basic dressings, phlebotomy (taking blood). Upholds ethical and professional standards and does not behave in a manner that is likely to bring the charity into disrepute.
