Our combined expertise in the fields of substance use, mental health, housing, criminal justice, and employment support enables Adferiad Recovery to meet the needs of our most vulnerable with a single, unified, and comprehensive approach.

**Job Purpose**:
This service will provide an accommodation and support service for Young People via a floating support model. The service will provide support that enables Young People to live independently and feel safe and secure within their own homes. The service will work with young people holistically, utilising Adferiad’ recovery model and supporting all aspects of a young person’s life - in order to achieve promoting wellbeing and a healthy lifestyle.
This service will deliver support to young people across the whole of the region; North, Mid, and South Powys. Support will be available between 8:30am to 9pm Mon to Sun with specific hours of support determined by each client need. Staff will be expected to be flexible and work across the hours and days of service delivery and will be part of a rota of on-call service for out of hours provision where necessary.
**Main Duties and Responsibilities**:
- Provide a variety of person centred, tailored interventions that support young people to manage their accommodation and develop independent living skills.
- Using a tailored support plan, develop opportunities to support a move-on to community engagement and vocational pathways using Adferiad’ s Recovery Programme as the basis for doing this.
- Build and maintain positive relationships with Young People to ensure successful support to address their accommodation and wider support needs.
- Provide timely and appropriate information, advice and guidance.
- Support young people to exercise informed choices about their lives where possible and to enhance their social skills and participate in their community.
- Support young people to identify the areas of support needed and identify the best option of support based on ongoing needs as outlined in their individual Pathway Plans.
- Provide support on a one-to-one casework basis and on a group basis such as delivering support groups and activities to promote peer support and social development and recovery.
- Completion of individual support/recovery plans, exit plans and other documentation as required, contributing to existing pathway plans where appropriate.
- Work with young people to achieve their outcomes and the outcomes of the project, employing creative and innovative methods to meet these outcomes and, through their experience and resources, bring added value.
- To assess service users for suitability taking into account risk and establish support plans based on established Adferiad Recovery assessment procedures.
- To provide a service to the young people that complements and enhances the service provision of the local housing providers in the context of the supporting people initiative.
- To participate in the provision of an on-call service.
**Full Driving License and access to vehicle is essential to this role.**
