Care Coordinator

Responsibilities
- 1. Creating and maintaining rotas using Birdie.
2. Provide leadership, management and coordination of the care delivery as per released Rota.
3. First point of contact for Service users and carers.
4. Monitor and maintain the electronic call monitoring system with check in and check out times.
5. Coordinating care packages, care plans, risk assessments, reviews, compliments/complaints, accidents and incidents.
6. Participate in the on call service on a rota basis.
7. Problem solving to allocate carers to calls which become uncovered at short notice (sickness etc). If required on rare occasions to deliver a care call.
8. Have excellent administrative and communication skills.
9. Be ensuring that new and existing care workers are supported and assessed throughout the induction process ensuring compliance, carry out spot checks and all quality audits.
11. Ensure communication is effective between management, service users, staff and professionals to maintain a robust quality service.
12. Attend meetings as delegated by the manager.
13. To maintain confidentiality at all times.
Pay: £12.75-£13.50 per hour
