Evening Reablement Care Worker

**About The Role**:
- Our reablement service provides short-term, high-quality care and support to individuals who are medically fit to leave the hospital but in need of further reablement services in their own home setting.
- You will be required to assess clients' needs to identify areas where support is needed to promote independent living and provide person-centred care.
- Motivating clients and building confidence so that they can achieve the goals they have set and continue to live independently is a key part of the role.
- You will be assisting with all aspects of daily living including personal care, encouraging the client to do as much as possible for themselves in line with their reablement goals.
- A **£500 welcome bonus **is payable upon completion of probation period, or after 6 months for internal transfers.
**About You**:
- Previous experience in caring for vulnerable adults with a wide range of complex needs is essential.
- Sensitive, caring, and passionate about delivering a first-class service to people within the local community.
- Proactive and self-motivated with great initiative and excellent communication skills.
- QCF Level 2 in Health and Social Care would be beneficial but is not essential as we can support you to achieve this qualification.
- A full driving licence and access to your own vehicle are required. You will need to be comfortable with lone working and travelling between customers.
**Hours**
You will be working on the below rolling basis;
**Week One**:
Tuesday: 4pm - 10pm
Friday: 4pm - 10pm
Sunday: 4pm - 10pm
**Week Two**:
Tuesday: 4pm - 10pm
Wednesday: 4pm - 10pm
Thursday: 4pm - 10pm
Friday: 4pm - 10pm
