**Job overview**:
The Keyworker Role is a ground-breaking 3 year pilot initiative, developed by NHS England and NHS Improvement Long Term Plan to improve outcomes for children and young people with Autism and / or Learning Disabilities, who are either in a mental inpatient setting or who are at risk of being admitted.

The role is unique in its development of a multi-agency commitment around the child to improve health, social and educational outcomes for the child and young person.

**Main duties of the job**:
The role is developmental and will evolve during the pilot, requiring a flexible approach. Functions for the role initially will be to ensure:
Multi-agency co-ordination around the young person’s Person Centred Support Plan, taking into account influencing factors including the wider family’s needs.

The voice of the child or young person (advocacy) is heard and accurately captured throughout assessments and reviews of the formulation of need and the care planning process,

Approaches from the assessment and formulation of strategies and plans (including positive behaviour support plans, communication plans, and environmental adaptations) devised by the clinical team are understood and consistently delivered across care settings including the home, education and care settings through intensive modelling.

Trouble-shooting to ensure the right help and consistent care, as set out in support plans can be implemented. This will involve direct contact with settings responsible for implementing the PBSP.

Escalation to the Team Manager where there are concerns the child’s emotional and mental health is deteriorating or complex behaviour is escalating, including working with NHS Kernow to instigate Care, Education and Treatment Reviews and review by the Complex Care Panel.

Knowledge and experience of safeguarding children and vulnerable adults and the ability to act in accordance with Trust procedures.
