**Job overview**:
This is an opportunity to join our developing Single Point of Access Community Mental Health Assessment Service.

SPoA Clinical Leads provide advice, clinical leadership, supervision and training to the SPoA team and are responsible for ensuring the consistent provision of high quality mental health care to people in community adult mental health settings.

The post holder will work with the senior leadership team to ensure the effective, efficient and sustained development of assessment services. The Clinical Leads will work closely with the Service Leads, Team Managers, Governance Team, Quality Lead and Operational Leads as well as our colleagues in Primary Care and Voluntary Sector services, to achieve high quality innovative care in the adult mental health settings with a focus on embedding the principles of reablement and trauma informed care.

**Main duties of the job**:

- Assess, plan, implement and evaluate individual care plans in partnership with service users, carers, and the health and social care community.
- Undertake comprehensive risk assessments and implement risk management strategies.
- Work closely with families and carers where appropriate to reduce psychological distress associated with the caring role
