We are looking for a highly motivated Social Care Practitioner** **to join our Rapid Response Service.

**About the role**

Our aim is to reduce unnecessary hospital admissions/carer breakdown and to promote independence by assessing people in their own homes and assisting people to maximise independent living.

This is an exciting opportunity to work for an Adult Social Care Directorate that innovates to achieve positive change for its residents and will give you the opportunity to stretch and grow.

You will work closely with Social Care colleagues and a developing multi-agency, integrated service combining Health and Social Care services.

You will undertake assessments of need to determine eligibility, implement care, and support plans with clear outcomes to achieve or maintain well-being. You will keep the individual at the centre of all activity and address assessed needs within the legislative framework.

You will be required to work over 7 days a week. Flexible working requests can be considered and will be discussed at interview.

**About You**

We are looking for a dynamic individual, able to work as part of a multi-disciplinary team; you should have a flexible ‘can do’ attitude to enable you to respond in a crisis, assess and provide care and support to people in their own home.

A good understanding of relevant adult social care legislation is essential, you should hold a Level 4 in Health and Social Care (or be committed to working towards this) and demonstrate previous experience of working in a health or social care environment or similar setting.

You should have the ability to work autonomously within agreed timeframes and manage frequently changing circumstances. You will be able to work as part of an integrated team, attend multi-disciplinary meetings and have excellent communication skills.
