At the Mental Health Worker you will work closely alongside the Band 7 Navigation hub team leader and the Senior band 7 Clinical lead to effectively triage referrals for Children and Young People presenting with Emotional Wellbeing and Mental health difficulties.

You will within the post facilitate good links between other agencies and CAMHS and signpost to other services where appropriate within the role of triage and ensuring that the referral needs are met.

The role will involves working as one of the clinical mental health practitioners in the team. The role will include delivering some direct clinical work, where possible jointly with other professionals as well as consultation for others.

To lead on the triage and acceptance of referrals direct from other professionals such as GP’s, Schools, Social care, and Children Young People organisations in order to signpost, refer and support appropriate and timely access to mental health services.

Responsibility of the day-to-day clinical management for the Navigation hub. clinical responsibilities to include patient, team activity, performance, quality and staffing matters.

To identify urgent and non-urgent referrals and respond accordingly within the Navigation Hub process to ensure the Child and Young Persons identified needs are allocated to the appropriate pathway /To work collaboratively within the Navigation Hub team with both admin colleagues and our partners from Coventry and Warwickshire Mind and Compass to provide a high-quality triage service to the Trust and other Health Care Professionals.To lead in the development of close working relationships with the referral sources by assisting in the development of clinical algorithms and pathways to ensure effective sign posting and pathways are maintained.Where necessary and clinically appropriate to make telephone enquires with the referrer, other identified professionals, and the Young Person/carer to assist with the triage decision and outcome.
