This is an exciting new role that develops collaboration between Independent Care Homes providers and TSDFT. You will be working with care homes and colleagues across health and social care to support people to find the right place for their care.

ou will bring your experience of working with care homes and hospitals to support collaboration and a focus on the person’s needs. You will be working independently to ensure that the assessment accurately reflects the person’s needs, and that care providers are supported to develop excellent person centered plans.

You will be working alongside the Torbay and South Devon Quality Assurance Teams. The QAIT teams are passionate about working in partnership with the Care Homes and delivering quality referrals. The team is flexible and promotes a healthy work/home life balance.
- To support development of a county-wide TPA model that contributes as part of the Integrated Care System, to timely transitions for individuals to residential/nursing care homes, from acute hospital and other intermediate settings and hence the effective delivery of the patient pathway and positive outcomes for individuals
- Support improvement in hospital discharge arrangements from hospital to Care Homes thus improve patient experience, clinical safety and patient discharge
- Promote person-centered discharge planning to Care Home settings, ensuring that relevant risk assessments, complex needs and patient information are all provided in detail as part of the discharge plan from hospital
- Facilitate discharge where issues have arisen which could compromise the quality or timeliness of discharge from hospital, working with all relevant staff across organisational boundaries with a problem-solving approach
- Act as a point of contact for ward staff/MDT/care homes, when residents are admitted to hospital from nursing or residential care home settings in order to monitor progress and keep on-going communications
- Acting as a conduit between care homes, Trust staff, and relevant others including DPT to support communications, considerations of assessments and individual patient and care home requirements to ensure safe appropriate discharges for existing resident or new resident being discharged to care home
- Liaising with and informing patient relatives of plans for care home transfer

The QAIT teams are supportive teams who enjoy mentoring team members and are passionate about working in partnership with the Care Homes and delivering quality referrals. The team is flexible and promotes a healthy work/home life balance.
