Exercise Physiologist

Work within professional competencies to provide holistic and responsive care and support to Wirral residents who have a cardiac diagnosis and meet the local referral criteria. Commit to a dynamic multi-professional service, driving forward person centred care, working together as part of the wider health and care system to reduce unwarranted variation, reduce health inequalities and improve access to CVD rehab that meets the individuals specific needs to ensure the best possible clinical and quality of life outcomes. They will also be responsible for facilitating Functional Capacity Tests and/or Exercise Tolerance Tests (depending on experience) and developing individualised exercise programmes. The post holder will also manage a clinical caseload, assessing the patient at entry to the programme and co-ordinating their care through to discharge.

Use reflective practice to build skills and knowledge in all aspects of early and core CVD Rehabilitation, plus working within the team to maintain national certification status and meet the 2023 BACPR Standards and Core Components. Please see attached Job Description and Personal Specification.
