Discharge Coordinator

An exciting new opportunity has arisen for an experienced individual to be based in Carseview supporting timely discharges. The post holder will be part of the wider Integrated Discharge Hub, which is also based in Ninewells Hospital and Royal Victoria Hospital.

You will require to be a Registered Nurse or AHP with a previous post registration experience.

You will support capacity and flow by providing support to patients to facilitate a safe and timely discharge home. You will undertake person centred assessments to identify and address health and social needs on discharge from hospital. You will work alongside multi-disciplinary and multi-agency teams across traditional, professional and service boundaries with the ability to adapt and manage change, and promote joint working.

You should have excellent communication and interpersonal skills. You should have recent experience in the assessment of individual’s needs. You must have the ability to make high level clinical judgements and have experience in problem solving/decision making in order to effectively manage patients’ health and social care needs.

You must be registered on the NMC Register or with the Health and Care Professions Council

Hours of work are: 37.5 hours per week.
