There is an exciting opportunity for a band 6 Palliative and End of Life Care Personalised Care and ReSPECT training project facilitator
The project has 12 months’ funding and the purpose of this role is to aid upskilling for personalised and Advance Care Planning conversations for Palliative and End of Life Care patients and support the adoption of ReSPECT. across OUH and aligning with stakeholders in the BOB ICS
The post holder will work with the project team to deliver the KPIs in the project plan.
To increase the identification of people who are likely to be in their last year of life through primary care, and to offer them personalised care and support planning conversations and to aid the adoption of ReSPECT
To increase the number of clinicians who are trained to initiate and support care and support planning conversations including DNACPR competencies
To identify inequity in groups of people who appear to be underserved in this way - because of diagnosis, ethnicity, social, economic or other factors
The Trust comprises four hospitals - the John Radcliffe Hospital, Churchill Hospital and Nuffield Orthopaedic Centre in Headington and the Horton General Hospital in Banbury.

Deliver Advance Care Planning/ReSPECT training and facilitate documentation to ensure best transmission of information within system capabilities
Support the design and delivery of personalised care training modules and ReSPECT aimed at identified staff groups and deliver training based on their needs
Develop relationships with Primary Care network Leads and staff in order to work with them to identify patients likely to be in the last year of life
Educate primary health care staff on best use of Palliative Care registers and meetings
Work across organisations to ensure engagement with the project aims
Manage information to contribute to reports and demonstrate achievement of Key Performance Indicators
Work autonomously within a wider health care team and as a lone practitioner.
Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health outcomes and healthcare delivery systems.
Work with ICB personalised care team to provide Palliative and End of Life updates
Identify gaps in training provision and work with the team to resolve
