**Hospiscare** is looking for **Care Navigator Volunteers** based in the **Community **across in the **Crediton **area

**About the **Care Navigator **role**:
Care Navigation, as part of the Supportive Care Team, supports patients and their families in a variety of different ways. The Supportive Care Team hopes to come alongside those in our care, as well as their families/carers to provide a holistic offer of support, recognising that people will find the support they need in different ways.

**You might be asked to**:

- Help with practical things including:

- Admin
- Digital support
- Help with benefits, form-filling and letter-writing
- Help with understanding aspects of the healthcare system
- Support with end of life planning including:

- Collecting memories and writing down life stories
- Talking about end of life plans
- Helping with funeral plans
- Facilitate activities that are important to patients such as:

- Shopping trips
- Visiting special places
- Provide a few hours of respite for carers
- Support patients with dementia
- Support patients and families during the last few hours of life

**We’re looking for people who can**:

- Be sensitive, flexible, empathetic and non-judgemental
- Use and develop good listening skills
- Be reliable and practical

Care Navigation is suited to people who have **experience in a caring role**, either professionally or personally. You will be supporting individuals and their families at home or in the community.

**We'll offer you all the training and support you need to be fully equipped to volunteer safely and effectively.**
