We have a fabulous job opportunity for you to join our awesome team as our Mental Health Support Coordinator. We support our brilliant customers who have complex mental health issues and challenging needs.
We help our brilliant customers by supporting them in their homes to gain independence, and the skills needed to move forward in the right direction. Our customers are all unique, with diverse needs meaning we need to adapt how we support them.
You’ll help change our customer’s lives (and our services) for the better by undertaking risk assessments, support planning, goal setting and regular reviews.
You’ll help our customers identify their aspirations, and work with them to create plans that help them achieve these, in collaboration with clinicians and wider NHS services. Each plan will look to meet our customer’s individual potential and long-term goals,making sure they meet safeguarding and confidentiality requirements.
As our Mental Health Support Coordinator, you may support our customers with breakfast, medication, attending college or the gym or taking part in community and social activities. You also may help with trips out, walking, and other meaningful activities, givingour customers choices, based on their needs.
You’ll be based at our mental health recovery service working with the team to provide support to our customers. It’s not always easy and our customers can be tough to support at times, however it’s a hugely rewarding job
