JOB PURPOSE The post holder will work within the Neurodevelopmental nurse led team, supporting the delivery of care to young people and their families who have a Neurodevelopmental Condition. SERVICE DELIVERY Demonstrate good organisational and communication skills whilst displaying a high standard of professionalism at all times, with an awareness of the need for confidentiality. Provide and receive routine information to inform, share as appropriate. Data entry, text processing or storage of data.

Advise patients and their families on how to contact the trust where they feel that there are delays in their pathway or require additional information. Utilisation of trust case note tracking system Manage workload and liaise with colleagues in periods of annual leave and sickness. Act autonomously in managing situations within scope of practice and know when to seek help. Provide information and signposting to other support services and liaise with clinical staff to support patients and families in distress.

Be adaptable, multi task and prioritise under pressure in a busy environment. Ensure that situations or enquiries regarding specialist interventions are referred appropriately and in a timely manner. Work collaboratively with clinical/non clinical services internal and external to the organisation to provide seamless service which is responsive to individual patient needs and liaise with all members of the extended MDT to aid and facilitate a collaborative approach to the care of the patient. GOVERNANCE Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care.

Comment on proposed changes to policies and procedures when required. Support and contribute to audit processes, governance, research and service development. Ensure adverse occurrences are recorded and reported via the trust incident reporting system in a timely manner. MANAGERIAL/LEADERSHIP Support the effective use of resources eg budgets, supplies and materials.

Ensure a healthy, safe and secure working environment, ensuring compliance with legal and regulatory requirements, maintaining accurate documentation and reporting any concerns. EDUCATION/LEARNING Support the delivery of patient and family training and education. Take responsibility for own learning and development by recognising and taking advantage of all opportunities to learn, including full participation in appraisal, supervision, action learning and by maintaining a professional/personal portfolio of learning. Demonstrate self directed learning, actively seeking role development opportunities to enhance practice, knowledge and role progression.

Identify personal education needs and skills development. CONTINUOUS IMPROVEMENT Support the trusts commitment to developing a continuous improvement culture by making everyday improvements as part of your job using improvement matters, the trusts single continuous improvement approach. Role model the behaviours of an improver displaying curiosity ,humility, compassion ,effective practice, open mindedness to try new ideas and self discipline. Take personal responsibility for the role you play in actively contributing to creating a culture which promotes learning as opposed to blame, and supports colleagues to embrace change.
