Paramedic Practitioner

We are looking for a Paramedic Practitioner who has completed the MSc Advanced Clinical Practice programme and has their Independent Prescribing qualification to work at The Border Practice. One of the four GP practices of Aldershot Health Primary Care Network.

This role will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. The post holder will demonstrate safe, clinical decision-making and expert care for patients within The Border Practice.

The PCN paramedic practitioner is expected to attend to patients within the defined list of conditions, including assessment, diagnosis and treatment to deliver quality patient services in conjunction with the practice General Practitioners.

The PCN paramedic practitioner will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures.

There is an expectation that training across the practice is maintained to an expected level with use of appropriate equipment and facilities as directed by the post holder who is an autonomous allied healthcare professional who will be part of PCN support to The Border Practice.

The post holder will use advanced clinical skills to provide education to service users, promoting self-care and empowering them to make informed choices about their treatment.

You will work closely with the practice and their team, as well as the PCN team of Clinical Pharmacists, Pharmacy Technicians, Social Prescribers and Care Coordinators.

The role will also include supporting the management of long-term conditions, working alongside the practice team.

The successful applicants will need to be self-motivated and capable of working closely with the practice team, networking with the wider PCN team, whilst also being able to work on their own.

Previous Primary Care working experience is desirable. The post is available as a full-time role, however part time of more than 25 hours per week, will be considered

You will have excellent organisational, interpersonal and communication skills together with a good working knowledge of primary care and the challenges facing medicines management in the NHS.
