**JOB DETAILS**

**Job Title: Social Prescribing Link Worker**

**Hours**:37.5 hours per week_(part time or job share applicants would be considered)_

**Responsible to**:Health & Well Being Team Leader

**JOB SUMMARY**

The role is to deliver a Social Prescribing Service to Oadby and Wigston Primary Care Network which includes:

- Bushloe Surgery
- Central Surgery, Oadby
- Rosemead Drive Surgery
- Wigston Central Surgery
- South Wigston Health Centre
- Croft Medical Centre

To receive, review and act upon referrals received from GPs and primary care staff. Referrals can be generated through proactive searches or through reactive referrals following patients’ interaction with the practice. An holistic assessment will be carried where required and individuals directed and sign-posted to the most appropriate provider of the services they need.

**MAIN DUTIES**

1. Undertake holistic assessments, in the GP surgery or at home if the patient is housebound, and co-design Health and Well-being Plans with individual service users, identifying support needs to ensure maximum engagement in improving health and well-being through face to face and virtual contacts, including within the individuals’ own homes where appropriate.

**2. **Provide service users with continuity and a coordinated experience of care, remaining point of contact throughout the individual’s social prescription**.**

**3. **Establish and maintain effective liaison and build relationships with stakeholders including health, voluntary, social and education resources, attending relevant meetings as necessary.

**4. **Work in partnership with voluntary and community organisations to contribute to a comprehensive and up to date database of local resources to design and support the Social Prescription Menu.

**5. **Support and develop GP’s and primary care health teams’ knowledge on how to identify patients suitable for social prescribing service referral.

**6. **Maintain comprehensive data and evaluation systems to feed into quarterly comprehensive outcome focused reports detailing the progress of the service.

**7. **Work closely with other members of the Health & Well Being team and wider PCN staff to ensure collaborative working

**8. **Keep records of your work and adhere to confidentiality, information sharing protocols and provide monitoring information as required using specific software.

**9. **Ensure you have an understanding (appropriate to your role) of, and comply with safeguarding procedures to protect the welfare of children and vulnerable adults.

**10. **Comply with PCN practices Health and Safety Policy, Data Protection Policy and to protect the health, safety and welfare of yourself and others.

**11. **Work flexibly as required by the service, including occasional Saturdays and evenings and to take part in Oadby and Wigston PCN and relevant organisations’ meetings and events to promote, support and celebrate the work of the service and the agencies

**12. **Work with the Social Prescribing team to further develop the project, taking in to account the changing requirements of patients, the community and surgeries within PCN.

**PERSON SPECIFICATION**

**Essential**

Educated to GCSE level or equivalent

2. A health, social care, local authority or voluntary sector background with suitable experience.

3. Demonstrate local knowledge of the services available in the area covered by the PCN.

4. Excellent holistic assessment skills as well as consultation skills and experience of providing empowering support to adults in a planned and structured way to improve health, recovery and well-being outcomes.

5. Proven skills in working with people who are experiencing low mood or needing support with low level mental health issues.

6. Able to provide support to people individually and in a group setting

7. Excellent record keeping skills and excellent written, verbal, listening and presentation skills

**8.** Good IT skills and ability to do own administration using data base, PowerPoint and other IT packages.

9. A confident and professional approach to working with a variety of stakeholders.

10. A proven understanding of safeguarding for children and vulnerable adults and ability to implement relevant policies and procedures.

**11.** The ability to work in a team and autonomously and to plan and prioritise work under pressure and adapt to new models of working.

**12.** A commitment to equal opportunities and an understanding of the impact on individual’s, families’ and communities health of deprivation.

**13.** A full driving licence and access to a vehicle.

**Desirable**
- Experience of using practice patient administration systems (EMIS Web and SystmOne).
- Ideally, completed the NHS England and NHS Improvement online learning programme hosted by Health Education England eLearning for Health platform and completed training as outlined by
