This post is fixed term/secondment for 12 months due to - meet the needs of the service.

**ACP role working against the 4 pillars of practice**:
Clinical - assessment/ diagnostics/treatment/ discharge/ complex communication with patients, relative and peers

Education - delivery of education to MDT around frailty, support mentorship to trainee ACPS and IPS (Multi-professional). Responsible for own development and education needs. Maintain live portfolio of evidence around AP.

Research - carry out Audit/QI/service evaluation/ research in relation to practice

Leadership - lead on service change, contribute to key HB strategy development groups

We welcome applicants who are looking for a job share and also ACP applicants from nursing, physiotherapy and paramedic backgrounds.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
