**Team Leader Job Description**

The purpose of this role is to support Homecare Scotland’s carers and clients in the community. Our group of team leaders lead by example, setting high standards for our services and the support we provide for our workforce. The role is a mix of delivering clients with care and of supervising and supporting carers. The Team Leaders reports directly to the Local Manager.

**RESPONSIBILITIES**
- Manage time effectively; working in conjunction with rota needs with a maximum 3 days a week delivering care with a minimum 2 days a week focused on monitoring and support for our Carer and Client community’s.
- To work closely as a team, in support of the Local Manager, to deliver and be able to evidence consistent quality of support and service. This may at times require flexibility to undertake tasks out-with of your usual area of responsibility.
- To ensure best practice across our Carer team in adopting our electronic care management system (Birdie).
- Care Plan Management: work with colleagues to ensure all clients have accurate Care Plans and risk assessment/reviewing these on a six-monthly basis, as circumstances require, or informally as opportunities allow. Care plans must be refreshed on our Care Management systems and communicated to carers where required.
- Rotate office-based administration day to ensure a Team Leader covers 1 of each of the days of Wednesday, Thursday and Friday, in support of the office team.
- Supervision: work with colleagues to ensure all carers have a supervision on a quarterly basis.

Two of these meetings should be observing the carer at work, the other 2 might be a face to face, formal or informal meeting. All records to be noted on Birdie and future planned dates entered to evidence schedule of rolling observations and support.
- Spot Checks: Work with colleagues to ensure all clients have at least 4 spot checks a year. These are unannounced client visits done whilst carers are present with the objective being to check all is well, client happy, carer on time, gloves & uniforms worn, id badge worn, care notes appropriately noted in Birdie. These visits must be logged on Birdie
- Deal with all health and welfare concerns by ensuring a record is made in Birdie, information is passed to the local manager, actions are agreed and or a concern raised with the appropriate authority.
- Ensure all carers have an annual appraisal.
- Ensure regular contact with fellow Care Team Leaders to review, refocus and adjust focus, as required with and by the Local Manager to respond to operational pressures. Advised, weekly meetings.
- Be familiar with Care Inspectorate guidance, current focus areas, observe national care standards and take responsibility for the monitoring and recording of information that will support our evidence of achieving these standards.
- Support week night (occasionally), weekend on call (on a shared rota basis) ; on these weekends the T/L will have 2 days off that week. She will work as normal in the community doing spot checks, supervisions and care if needed. The team leader will always be supported by a colleague who will be taking calls/whenever possible
- Oversight and responsibility for ensuring our commitment to reviewing, managing, and adopting industry Leading practise in support of our clients, carers and use of our Care Management Systems (Birdie). Each Team Leader will have a lead responsibility with one of these areas (as noted above) and should be able to evidence our current quality indicators and progress made against targets set.

Non but support of carers within their set geographical location - carers report directly to Local Manager

**Administrative Responsibilities**
- Responsible for ensuring that care management systems (Birdie) are accurate and updated with all care plan information, carer supervisions and spot checks.

**Salary: £26k + weekend on call payment**

**Salary**: £26,000.00 per year

**Experience**:

- providing home care: 3 years (preferred)
- supervisory or team leader: 1 year (preferred)
- creating and reviewing care plans: 2 years (preferred)

Licence/Certification:

- Driving Licence (required)
- NVQ Level 2 Health & Social Care (required)

Willingness to travel:

- 50% (preferred)
