**Who are we?**

We are a local charity with a big heart and have been providing exceptional care and support to people affected by dementia for over 30 years. We are the local specialist dementia service and empower people living with dementia to carry on their chosen lifestyle. With a variety of support services, we equip people with the knowledge and practical support to live well with dementia.

We operate differently to a lot of other Care providers, in the sense that we will NEVER expect you to cut a visit short to get to the next. You're paid an hourly rate for your TIME not just for the visits you do, and your mileage is paid on top at 45p per mile.

We're looking to recruit Community Care & Support Workers in and around the Dartford, Gravesend and Swanley area for the weekends with guaranteed hours.

**About the role**

Our Care Team are critical to us achieving our mission to give people affected by dementia the compassion, support and the guidance they need to live their lives the way they choose.

We provide our Team with an extensive induction programme that covers all mandatory topics and deep insight into how to provide truly person centred care. We will ensure that you are confident and competent in supporting people with all aspects of their daily lives - and what a privilege this can be

This role will require a Enhanced DBS check, for which we will cover the cost.

**Main Responsibilities**:
1. Provide care and support with all aspects of daily living; this could include personal care, medication administration, shopping, going out for the day, emotional well-being or light domestic tasks.

2. Actively promote informed choices decision making, inclusion and emotional well-being - enabling people to remain independent in their own home and their local community.

3. Ensure you to strive to provide the highest level of care and comfort to the people we support, against agreed our standards.

4. Ensure that all monitoring processes are followed and that accurate, factual records are kept.

5. Work in a compassionate, empathetic and caring way; affording the people who use our service the highest levels of dignity.

6. Work with the Senior Support at Home Team to achieve objectives and outcomes within agreed parameters set.

7. Actively promote and protect confidentiality and dignity in all areas of the organisation.

8. Positively engage with training, supervisions, team meetings and appraisals and contribute to the development of the service.

9. Work in accordance with Alzheimer’s & Dementia Support Services policies and procedures and act as an informed advocate for the Charity.
