An opportunity is available for an experienced Mental Health Practitioner to provide specialist mental health care for the newly developed Integrated Placed Based Team.

The post is based in South Lincolnshire Rural locality covering the Primary Care Networks, Moulton, Gosberton, Sutterton, Long Sutton, Holbeach, Littlebury, Galletly, Deepings & Abbeyview.

The role gives excellent opportunity for co-production with Lincolnshire health, social and voluntary care system, patients, and carers. This not only ensures appropriate, and needs led outcomes for the patients, but also offers the practitioner opportunities for creative approaches to working and ensuring the patient remains central in the care and intervention planning. This enables individuals to achieve their optimum physical, psychological, and social wellbeing and improve the lives of people who access healthcare services.

A comprehensive trust induction will be offered and opportunities for CPD. Management and Clinical supervision are provided regularly.
