Discharge Coordinator

The SRH Discharge Team is looking for two new coordinators to join us in providing a high quality discharge service to all on the SRH site.
If you have an interest in discharge planning and want to take on this challenging and rewarding role, we want to hear from you.
The SRH Discharge Team is a friendly, supportive and hard-working team comprising 12 coordinators (including yourself), two secretaries and a senior coordinator.
Currently some of the team are undertaking PFIS training and the team hopes to use the tools of that system to further improve the service provided.

- Act as key-worker to co-ordinate individual complex discharges
- Act as Trust experts in discharge planning, assisting and supporting the MDT to drive and co-ordinate discharges, ensuring effective, timely intervention from all disciplines.
- Assist wards and teams with understanding and using the ‘Discharge to Assess’ model
- Act as Trust experts for referrals for NHS funding for Care Home
placements and act as the Trust’s liaison with the CCG CHC Team
- Support the integrated ‘Discharge Hub’ in matching patients to
pathways
- Provide education in discharge planning to members of the MDT or
other professional groups requiring this teaching
- Support transfers to NHS funded community venues/services
- Participate in the development of Trusted Assessment for care
transfers
