We are excited to announce that we are recruiting another Band 4 Assistant Practitioner to join our Community Intermediate Care Centre.

This post is to support the Discharge to Assess Team. This post is fundamental in supporting clinical delivery and flow across the system in aiding people to regain independence andfunction with the view to determine long term discharge destinations.The post holder will work as part of a designated clinical team delivering health care that focuses on the direct needs ofthe patient. They will carry out specific clinical tasks and responsibilities as identified by registered practitioners andaccept clinical responsibility for a caseload where appropriate.In addition, the post holder will develop and maintain knowledge, skills and competences, under supervision, related to therole of Assistant Practitioner.The assistant practitioner will work as part of a multi-disciplinary/integrated team. The role will work across Physiotherapyand Occupational Therapy boundaries providing support to both professions
