**Commissioning Care Packages for Specialised Cases**:

- Commissioning bespoke care packages for individuals with Learning Disabilities, Physical & Sensory Disability, Dementia or Mental Health issues.
- Implementing suitable care packages with Reablement and homecare providers under the Domiciliary Care Framework Contract, ensuring prompt, secure, and efficient handover of clients between providers.
- Commissioning Respite and Long Term Residential/Nursing Home Placements for clients, including smooth transfers to chosen facilities.
- Developing market intelligence and documenting it for understanding of reasonable rates and quality of provision, and to enhance knowledge of the post holder.
- Collaborating with the Placements team to ensure accurate and timely recording of placements on relevant systems and monitoring of financial records aligned with applicable IT systems.
- Collaborating with the Quality Service to address issues related to quality of provision.
- Maintaining a comprehensive and accessible record of services, and individuals placed, including dates, reviews, and costs.
- Supporting identification of placements with suboptimal outcomes, and working closely with operational colleagues to challenge provision that is not as supportive or independence-building as it could be.
- Identifying and addressing gaps in service provision, linking them to appropriate placements to optimise capacity.
- Work with operational managers and practitioners across all social care services to identify clients with complex needs who require specific placements and commission appropriate placements.
- Collaborate with providers to deliver homecare services, residential care, supported living, and other placements to clients of the borough.
- Ensure that all clients are placed with the appropriate service, and relevant documents are securely shared with providers.
- Accurately record all related documentation electronically. The post requires the ability to find solutions to difficult placements and to deal diplomatically with family members and providers. Ensure accurate recording of all placements in the Adult Social Care Swift system and timely issuing and monitoring of all contracts associated with package placement.
- Liaise with care managers, home care reviewers, and assessors to ensure that changes to care packages as identified in assessments or reviews are actioned promptly.
- Prioritise the placement of individuals requiring discharge from the hospital. Possess a comprehensive understanding of standard contracts for placements in all provisions.
- Frequent contact with care managers, service managers, providers of ASC services, performance departments, shared services, care homes, service users, and families or guardians.

**Requirements**:

- Of working in a brokerage/ commissioning function.
- Of dealing with external providers for the provision of care services.
- Of working within Social Care
- Of dealing sensitively with issues that can reach across many stakeholders, including providers, families and operational services

**Working conditions/circumstances
- Office based

Requirements A bachelor's degree or higher in Social Work and registration with the relevant professional regulatory body are required. Knowledge of the legislative frameworks governing child and family social work is essential, as is experience working in a statutory setting. Understanding the impact of different parenting styles on child development and the development of resilience in the face of loss, change, and uncertainty is crucial. Some experience working in a childcare setting, either through student placement, voluntary work, or paid employment, is preferred. Additionally, the ability to assess risk and need, plan and review outcomes, and work with service users who exhibit challenging behaviour is necessary. The ability to recognise signs of various forms of harm to children, including physical, emotional, and sexual abuse and neglect, as well as the consequences of cumulative harm, especially in terms of early indicators of neglect, is essential. This role also requires strong communication skills, both written and verbal, as well as the ability to use technology effectively. The ability to build and maintain relationships in challenging situations such as those involving resistance to change, ambivalence, or selective cooperation with services is also necessary. The role requires excellent time management skills and the ability to prioritise competing demands. Additionally, the ability to use strategies that promote professional resilience and self-management in challenging situations is important. The job involves working alone in a home environment
