We are seeking to recruit an experienced, competent, and motivated Therapy Senior Support Worker to work in the new Day Therapy Unit based within St Johns Hospice.

Part of this exciting new role would be to support the Day Therapy Unit team in the delivery of the new Therapy Service promoting a rehabilitative palliative care model. You will be a knowledgeable and forward-thinking individual who is able to provide prescribed structured education, exercise, support and guidance to patients.

The St Johns Day Therapy Unit will integrate rehabilitative palliative care with a specialist palliative care model to focus on Advance Care Planning, management of symptoms, rehabilitation, enablement, self-management, and self-care.

This role aims to support, promote and embed an individualised patient centred approach for palliative patients attending the Day Therapy Unit. The Therapy Team will support palliative patients, at any stage of their illness, from diagnosis, through to deteriorating advanced disease.

To work independently delivering agreed activities with patients / service users with access to senior staff for support when needed.]

Supervision of patients whilst in group sessions or 1:1 sessions

Review and deliver exercise programmes - mobility/ strengthening and cardio programmes within both group and 1:1 environment

Review and progress mobility programmes, assessing for and reviewing with equipment.

Deliver and progress relaxation techniques.

Deliver patient education both within group sessions and 1:1

To contribute to the running and organisation of a group session

To assist patients with appropriate tasks whilst attending the unit (e.g. toileting, obtaining food/drink)

Review of patients using standardised therapy assessments where indicated.

To report progress of the patient and changes in condition to the appropriate person.

To ensure accurate and timely documentation of care on SystmOne

To develop clinical competency and skills through the completion of competency-based training, and to maintain skills through regular re-assessment and training.

To demonstrate sensitivity to the patient and carer’s needs by respecting privacy and dignity, whilst always maintaining professional boundaries.

Develop a rapport with patients and encourage them to participate in the programmes and activities
